Para la exposición de las características clínicas del juego patológico, vamos a seguir a Fernández-Montalvo, J. y López-Goñi, J.J. (2014)[1], quienes las agrupan en tres áreas: fisiológica y emocional, cognitiva y conductual.
Área fisiológica y emocional
• Los pacientes experimentan una excitación y/o activación fisiológica relacionada con la conducta de juego.
• Pueden presentar problemas de sueño.
• También podemos encontrar síntomas depresivos y angustia generada por las pérdidas económicas y el pago de las posibles deudas que se deriven de la conducta de juego.
• El cese de la conducta de juego (o al menos el intento de abandonarlo) provoca irritabilidad y/o nerviosismo.
Área cognitiva
• A nivel cognitivo, el paciente va a presentar preocupaciones por el juego y las pérdidas asociadas a este.
• Podemos observar distorsiones cognitivas y sesgos: puede negar su problema, minimizarlo, presentar ilusión de control (considera que controla los resultados del juego de azar), estrategias de juego, etc.
• Puede aparecer ideación suicida.
Área conductual
• En primer lugar, el paciente dedica mucho tiempo a la conducta de juego.
• Un síntoma que recuerda a la tolerancia que se produce en las adicciones relacionadas con sustancias y que está recogido en el DSM-5 consiste en que la persona, para conseguir el nivel de excitación que desea, apuesta cantidades cada vez mayores de dinero.
• El paciente sufre pérdidas económicas derivadas del juego, lo que le puede llevar a jugar de nuevo para tratar de recuperar el dinero perdido. Estas pérdidas dan lugar a deudas e impagos constantes. Todo ello provoca una espiral de la que le cuesta salir al paciente. Esta situación puede también llevar a la persona a robar o realizar actos delictivos.
• Toda esta situación lleva también al paciente a mentir constantemente.

CAUSAS DE LA ADICCIÓN AL JUEGO
Existen distintas teorías explicativas para el origen del juego patológico, aunque no parece haber evidencia de la superioridad de una frente a las demás.
• Psicología conductual: esta establece el inicio de la adicción al juego mediante el reforzamiento positivo que dan los premios (aunque no supongan premios muy grandes) y pasa, una vez perdido el control, al refuerzo negativo, esto es, la evitación del malestar por no jugar o la angustia provocada por las pérdidas económicas.
• Mecanismo de ejecución conductual: esta teoría propone que, ante estímulos relacionados con el juego, el paciente siente la necesidad de "completar la conducta" (es decir, jugar) ya que, si esto no sucede, la persona experimenta malestar subjetivo.
• Activación psicofisiológica: estas teorías postulan que el reforzador que hace que la conducta tienda a repetirse sería la propia activación fisiológica que provoca el juego.
• Modelos cognitivos: estos modelos proponen la existencia de distorsiones cognitivas en el desarrollo y mantenimiento del juego patológico. Se han encontrado sesgos cognitivos tales como ilusión de control, evaluación sesgada de resultados, supersticiones o recuerdo selectivo de las ganancias.
• Modelos biológicos: desde el campo de la biología también se ha tratado de explicar qué sucede en los jugadores patológicos. En esta área se ha estudiado la disminución de neurotransmisión tanto de serotonina como de noradrenalina y dopamina.
